Common causes:

  • You’re running the RTM (release-to-manufacturing) build, and service packs have come out
  • You’re on an old service pack that Microsoft no longer supports
  • You’re on a version of SQL 2012 or 2014 that has an index corruption bug

To Fix the Problem

Usually the best bet is to get the latest Service Pack and Cumulative Update. Our favorite resource is SQLServerUpdates.com.
