Common causes:

  • You’re running the RTM (release-to-manufacturing) build, and service packs have come out
  • You’re on an old service pack that Microsoft no longer supports
  • You’re on a version of SQL 2012 or 2014 that has an index corruption bug

To Fix the Problem
